If you are buying a custom plan, please fill this form out and include the following in addition: Please tell me what your goals are, how many times you want to workout per week, what type of training split you want, and if you have any particular exercises /lifts that you don't want in the plan, ex: (squat, bench deadlift, etc). Please also let me know if you have any physical conditions or injuries that I should be aware of (anything that prevents you from performing any given lift).
